HOW DO I CHANGE MY ADDRESS?

• On the back of your precinct card, sign and mail
• At the Board of Elections Office, Public Libraries and City Halls, complete a change of address form
• Download an application from the Secretary of State's website, complete, sign and mail

PLEASE NOTE: No change will be processed without a signature.

HOW DO I CHANGE MY NAME?

• You must fill out a new registration application showing your new and old name. An application may be obtained from the Board of Elections Office, Public Libraries and City Halls or an application may be downloaded from the Secretary of State's website.

PLEASE NOTE: No change of name will be processed without a signature or if done on the back of a precinct card.

All changes must be received and/or postmarked by the US Postal Service 30 days prior to an election.
